1998 Volkswagen Golf Problems - Transmission and Clutch Concerns

I've got a 1998 Volkswagen Golf with some concerning issues and I'm hoping for some advice. The car is having trouble shifting gears, sometimes jerking or slipping into neutral, and the clutch pedal seems to be getting spongy. Has anyone else experienced these problems? What could be causing them and how can they be fixed?
